Agreements + Signature

I certify that no communication of publication, written or verbal will contain information which is not allowable under IRB review policies. This applies to all biomaterial obtained from the BioSpecimen Core Laboratory.

I agree that the tissue of biomaterial provided by the Biospecimen Core Laboratory will be utilized in the scientific experiments as described in the above sited protocol(s). Tissue or biomaterial shall not be traded or distributed further to third parties without the written consent of the Tissue Utilization Review Committee.

I understand that tissue or biomaterial obtained from the Biospecimen Core Laboratory could contain infectious or biohazardous materials. The Biospecimen Core Laboratory is not responsible for any injury, damage, or loss that may arise from the use of such tissues and/or biomaterial. I as the Principal Investigator recieving this tissue or biomaterial, agree to assume all risks and responsibilities in connection with the handling, storage, use, and disposal of the tissues and tissue derivatives.

I agree to acknowledge the contribution of the Biospecimen Core Laboratory (BCL) and the Louisiana Cancer Research Center (LCRC) in all publications resulting from the use of materials obtained from the facility. The facility should be acknowledged as follows:
a. Materials Section: Tissues were obtained through the Louisiana Cancer Research Center Biospecimen Core.
b. Acknowledgements: This work has been supported in part by the Biospecimen Core Laboratory of the Louisiana Cancer Research Center.

I understand that the TURC meetings are held every 2nd Monday of the month and my application should be completed by the Monday prior to the meeting.

I understand that fluid specimens will be released to my project within a week of TURC approval. Delivery of these tissues may be dependent upon the Tulane Histology Service schedule.
